Fix missing lsp commands

What has changed?

Added 'replace-refer-all-with-alias' & 'replace-refer-all-with-refer' actions to calva.

I scanned through all the commands in lsp's code_actions and these two were the only ones missing, added and tested.

Fixes #2667
