Upgrade application to run on Hyperledger Fabric v1.2

Make this code compatible with the following Fabric components:

• Fabric: https://github.com/hyperledger/fabric/tree/release-1.2
• Fabric-CA: https://github.com/hyperledger/fabric-ca/tree/release-1.2
• Fabric-SDK-Node: https://github.com/hyperledger/fabric-sdk-node/tree/release-1.2

Unfortunately, I haven't had any time since my initial (and partial) port, which only covered the network part. The middleware is yet to be ported. I might revisit this in the second half of September, time permitting.

You are welcome to take a shot if you want. Submit pull requests and I'll review and test.
