idxtool

The idxtool can be used to generate TOC, update GPT file(s), etc, etc.

So far, some functionalities are WIP.

In general, it should be called from the TOC generator BASH script.

idxtool will return 0 on success and 1 on failure.

Other changes

• Re-organized TOC.md so it works better with idxtool
• Update the TOC.md as it is automatically generated now
• Updated the project's README.md (rephrasing, better explanations, etc.)
• Fixed other broken GPTs (probably through yesterday's automatic formatting).
