- Replace custom instructions and constraints files with AGENTS.md
- Implement automatic AGENTS.md detection via AGENTS_FILE constant
- Simplify system prompt setup and user message processing
- Remove constraint appending logic
- Clean up unused working directory retrieval in websocket server
- Update configuration templates and reference documentation

BREAKING CHANGE: custom_instructions_file_name and custom_constraints_file_name are removed from config.toml
- Rename INSTRUCTIONS.md to AGENTS.md
- Bump octolib to 0.25.0
- Update rand, regex, and crossbeam dependencies
- Handle usage return values in embedding providers
