https://github.com/matrix-org/sydent takes care of 3rd party identifiers, 3pIDs, and maps phone numbers, Skype accounts, email addresses and more to Matrix homeserver identities.

Nowadays it is generally recommended to run https://github.com/kamax-io/mxisd for one own.

matrix-org/sygnal#13 (comment) states it is simple to run ones own sygnal 3PID service.
