version 3 - SMART Web Messaging

The app no longer relies on an external service for evaluating auc logic - that code has been refactored and modularized. The app can also communicate with the launching EHR to update pending orders, automatically inserting evidence that guideline consultation had been completed.

version 2 - enables SMART launches

The old login mechanism has been replaced with a SMART launch and the form is pre-populated with the selected patient and provider context.
