I've also added redirects for old links - those are configured in the admin panel, rather than in code.

Remove the old readthedocs site and set up a new one using mdbook on GitHub Pages. Also fixes a bunch of formatting issues in the CHANGELOGs which I never noticed before but which are now causing problems with a different parser.
