docs(licence): carry the dated change notice GPL-2.0 asks for - #1

This fork goes public, so section 2(a) starts to bite: a modified file has to say that it was changed and when. INJECTIVE_FORK.md already listed every changed path; it now states the date and the terms, and the one upstream file we edit carries the notice itself.
