Re-pins the shared review workflow onto dashpay/stale_prs_are_bad#46.

The checklist, in the description. Every governed pull request's description now ends with a block the controller owns — every requirement, checked when met, the first unchecked line being the state. Your own text above it is never touched. GitHub counts the boxes into the "N of M" bar in the pull request list.
