- Notify command interceptors about provider reader failures. - Cover query reads, cancellation, and SaveChanges result advancement. Fixes dotnet#35229
- Include failure and cancellation logging consumers when enabling reader reporting. - Cover buffered reads, result advancement, materialization boundaries, and SQLite overflow.
