Refuse foreign PR URL hosts, add pr ready and more merge options

greetings and thanks for writing this! I'm using it with Gitea and firstmate. Just a few patches:

• A PR URL whose host differs from the configured base URL is now refused with HOST_MISMATCH instead of being read from the configured instance.
• New pr ready NUMBER|URL takes a PR out of draft by removing its WIP/Draft title prefix (idempotent).
• pr merge accepts --method rebase-merge and --method fast-forward-only, plus --delete-branch.
