add: anchors to includes map

Added in this pull request:

• record anchors in the includes map, that become available after including;
• adjust of links in the included content depending on parent file, added tests;
• cropping the content before manipulating the content, for instance, the escapecode is not applied to an entire file but only to a part that will be included.
