fix: honor disabled site rules in embedded frames

Summary

• Apply disabled site rules to the current frame and its ancestor origins.
• Prevent the Video Speed Controller from initializing inside an embedded player when the enclosing site is disabled.

Closes #1634

Validation

• Focused content bridge and site pattern tests: 37 passed.
• Direct ESLint checks pass for all source and test JavaScript.
• Prettier check passes for both changed files.
• Release build passes with RELEASE=1 node scripts/build.mjs.
• Full test suite: 552 passed; two release-helper tests cannot run on this Windows environment because unzip is unavailable.
