Batch of small doc changes to bring the older tier CHANGELOG
entries and the guardrails maintainer README into a consistent
plain-prose style.
Changes:
- CHANGELOG Tier 10 through Tier 12 entries rewritten in plain
Keep-a-Changelog prose to match the Tier 13-18 style
- guardrails/README.md rewritten to reduce table-heavy structure
and remove em dashes
- Light em-dash sweep on docs/ROOT_CAUSE_AUDIT.md,
docs/cais_state_machines.md, and guardrails/VERIFICATION_PROTOCOL.md
- .gitignore extended to hide the local push_*.sh helper scripts
No content coverage lost. Docs-only patch; no code, DDL, or
allowlist changes. All 8 guardrails still pass with 0 field-mapping
allowlist exceptions.
