[kernels] CUDA version of block_sparse_attn

This PR introduces block sparse attention kernel to our SeerAttention using CUDA/CUTLASS, resulting in enhanced computational efficiency compared with previous Triton-based one.

• Add this kernel in modeling_llama_seerattn.py
