docs/SKILL.md documented a nonexistent scan command, --output/--baseline flags, and a JSON schema that never shipped. Rewrite Commands, JSON output, Exit codes, Failure Modes, and Parsing examples to match aws/gcp/init/version exactly, verified against source. Also add the 0.1.2 CHANGELOG entry summarizing the WO-4..13 fixes merged in PR #1.
