This number is the maximum you can have in the game without any special mapping technique. Otherwise you could go up to +-100k with some techniques.
Recently people found out that they can play very big maps in 1.6. There are a few servers supporting this change already, notably Surf Gateway (surf) with lots of CSS never-able-to-be-done-before ports and Romanian Jumpers (Kreedz climbing) where I personally dump a lot of my stupidly huge maps.
For reference, CSS maps are +-16384 big.
Everything should work like the player and entities can freely move outside +-4096 range and players can spectate each other just fine. The only things not working are unforunately engine hardcodes such as temp entity or sound (not indexed by the listener).
