Add new FATES crop PFT and HLM PFTs

Description:

Combine various changes to parameter file. This brings in changes from PR #53 and also adds the two extra HLM PFTs to go with the 16 hlm PFT surface file. Diff looks much bigger than it is, I think variable order is getting mixed up from FatesPFTIndexSwapper.

I guess Charlie's tool reorders the variables, hence the change in the order of the headers? I saw this when I tried to do this earlier as well. Otherwise this looks as intended.
