QA: replay fresh-default review-app deployment

Purpose

Disposable fresh-default diagnostic replay for issue #194, following the failed disposable replay in #196. This PR exists only to exercise the default review-app lifecycle.

Scope

• exact parent: current main at the verified default commit
• change: one inert marker file
• no application, deployment, workflow, policy, or credential configuration changes
• PR Fix review app database credential mapping #197 remains untouched

Verification plan

• trigger the default review-app deployment once
• record sanitized pass/fail/UNKNOWN lifecycle evidence
• close this PR unmerged and verify cleanup
• remove the remote diagnostic branch after cleanup

This PR is related to #194 but must not close it automatically.
