Summary

ignore compiled JS in examples
run createFetch with a base URL to avoid Node errors
mark the example as an ES module

Testing

pnpm --filter openapi-hooks build
pnpm --filter basic dev
node examples/basic/src/index.js (fails to connect but no module errors)
