Add work item relation linking tool

Summary

• add link_work_items MCP tool for parent, child, predecessor, successor, and related links
• add service support for applying work item relation JSON Patch updates
• document the new tool and add unit coverage

• dotnet test Solution.slnx with temporary NuGet config
• dotnet format Solution.slnx --verify-no-changes --no-restore
