SHH HC executioner's bug

Here you can see that the mobs who should be waiting for the execution are missing (including the executioner himself) whilst we all have the buff for 30 more minutes till FIRST execution.

This bug is due to server crashes ( happened to me once, but a lot of people actually warned me that it happens, so it's not uncommon). As soon as the executioner's timer is initiated (after you kill first boss) you begin a game of russian roulette. If you nuke the instance BEFORE CRASH, you get your TK attune, 5th badge, etc. If server CRASHES however BEFORE you managed to do it, no executioner will appear.

So I am both asking to fix the bug, and if possible to help the following players with the quest item(since we had done everything right and we would have finished our Q had we had the chance to fight the executioner):
