Warrior charge is bugged

Zone/Instance: Every zone in Outlands so far.
What type of error occurred?: Falling through the ground while charging
What was the actual result vs. the expected result?: While charging anything, I fall through the ground. The expected result would be that I charged the target, not falling through.
Can the error be repeated? If so, what are the steps?: Yes, use a warrior, target something in Outlands and charge it.
Please detail your character’s race, class, and items (optional): I’m an undead warrior, Raggmunk, not that it would affect this issue
