Add per-tool removal and clear-all actions to recent tool cards and sidebar menus. Persist changes and refresh the sidebar and All tools page while preserving favorites, tool settings, ordering and the three-item limit. Hiding recent tools continues to preserve history.

Cover empty history, reopening tools, active-page updates and reloads from disk. All 168 tests pass in Release configuration.
